Northwell Health is the sole provider of the license for the DECIDE™ Self-management Program.  License fees are negotiated based on the number of participants the organization intends to reach and are term limited.  Only personnel from licensed organizations may complete the DECIDE Facilitator Training and become certified to deliver the DECIDE program.  The license agreement specifies the number of DECIDE Facilitators that will be trained.  However, training costs are not included in the license fee and are based on the cost of training delivery (e.g., virtual or in-person) and the number of people to be trained.
