DECIDE Program

DECIDE is flexible for delivery in a variety of formats to suit the differing needs of organizations and individuals. You can choose a program version for your population's preferences and needs.

DECIDE In-Person Delivery

The DECIDE In-Person is delivered by a trained DECIDE Facilitator in group or one-on-one formats. The program is delivered over a series of 9 sessions (one session per content module) and uses a DECIDE Facilitator Guide and a hard copy DECIDE program participant workbook. The In-Person delivery can be conducted in a variety of settings including health care practices and community settings. Facilitators must complete DECIDE Facilitator Training and certification to be eligible to deliver the program. 

DECIDE In-Person Program

DECIDE Virtual Delivery 

Health care and public health organizations providing virtual and digital communication services can provide the DECIDE program in real time to program participants through this remote access. A DECIDE Facilitator conducts the series of 9 DECIDE program sessions live, with program participants accessing the program via a virtual delivery platform (e.g., Zoom). Virtual delivery has the advantages of DECIDE in-person delivery, scaled to large numbers of participants who can attend from convenient locations.

DECIDE Self-Directed Digital Delivery

DECIDE Self-Directed digital delivery, users can navigate through the 9 program modules from anywhere, and at their own pace. The program is narrated in English and Spanish with an option for closed captioning and a screen reader.

DECIDE for Multiple Chronic Conditions

The structured program format enables DECIDE to serve effectively as a platform for patient self-management training for any chronic condition. Module 1 is self-management education based on current standards of care and clinical guidelines specific to the condition. Modules 2-9 provide a standardized problem-solving training approach that applies across conditions. Condition-specific versions of DECIDE are diabetes, high blood pressure, chronic kidney disease, and heart failure. Versions can be combined to support participants with multiple chronic conditions.
